Requirements for a State Training License needed to start ResidencyAlabama
The State of Alabama does not require licensure for physicians who are participating in one of the approved residency training programs.  I suggest that you contact the program that you are interested in and inquire as to what their rules and regulations are concerning licensure.  Most of the programs require you to have a limited license but those licenses are requested by the residency programs.

The Arkansas State Medical Board does not have a “training license”.  As per our policy, you do not need a  license if you are in an internship or residency training program, unless you are moonlighting or working outside of the training program.  If that is the case, then you will need to apply for a permanent license.   Please refer to our website at www.armedicalboard.org for requirements for M.D./D.O. medical license and application packet.

Of the total 72 weeks of required clinical training, 54 weeks of training must have been completed in hospitals that satisfy one of the requirements in Section 2089.5 (d).  The required core clinical clerkships are listed in subsection (c).

Licensure is not required for participation in residency programs.  Permits are issued to the teaching hospital on behalf of  the residents.  One you have been accepted into the porgram, the hospital is responsible for obtaining the residency permit.

There are two essential credentials required for postgraduate training by physicians who were educated outside of the United States.  First, the program must be approved by the Accreditation Council for Graduate Medical Education (ACGME).  Second, you must have a certificate issued by Educational Commission for Foreign Medical Graduates (ECFMG).  The ECFMG certificate involves approval of your educational program and passing examinations in basic sciences and clinical theory.  The ECFMG can be reached by phone at (215) 386-5900.  You can check whether the program that you wish to enter is ACGME-approved by going on-line at www.acgme.org

Hello:  The information is in the rules and regulations and law.  The application is under “Forms”.  The hospital you would be working would be processing the application and sending it to me.
You would need to apply at a hospital that offers an approved ACGME training program.  You would need to obtain 3 years of post graduate training.  You would have to apply directly at the hospital and they would give you an application to complete and the institution would send it to me and I would issue a training license.

Hawaii

We do not have any rules or requirements regarding clinical rotations during medical school.

Iowa
To be eligible for a resident medical license you must
1. Be at least 20 years of age

2. Hold a medical degree from an educational institution approved by the board at the time you graduated and was awarded the degree.

a. Educational institutions approved by the board shall be fully accredited by an accrediting agency recognized by the board as schools of instruction in medicine and surgery or osteopathic medicine and surgery and empowered to grant academic degrees in medicine.

b. The accrediting bodies currently recognized by the board are the Liaison Committee on Medical Education and the American Osteopathic Association.

c. If you hold a medical degree from an educational institution not approved by the board at the time a degree was awarded, you will need to have a valid ECFMG certificate or have successfully completed a fifth pathway program.
We do not have a clinical rotation requirement.

In order to apply for a training license in Massachusetts, you will first need to be accepted into a training program. We require that graduates provide a competed E-1 form and E-2 verifications from each rotation. These requirements are defined in our Board Regulations, which can be found at 2.02 here:

Michigan
You can get the application pack for the educational limited M.D. license at www.michigan.gov/healthlicense  On the left open the medicine page and scroll down under the FAQ section for the application.  We require that the program was not less than 130 weeks and does not award credit for any courses taken by correspondence.  The medical education program must include basic science courses in anatomy, physiology, biochemistry, microbiology, pathology, pharmacology, and therapeutics, and preventive medicine, and clinical science rotations in internal medicine, general surgery, pediatrics, ob/gyn, psychiatry. All must be done in at a teaching hospital.  Teaching hospital means they offer a postgraduate clinical training program in the same content area of the clerkship.  For more information refer the Certificate of Medical education form that is part of the application pack.

Nebraska
The requirements for a foreign medical graduate to obtain a Temporary Educational Permit in Nebraska are to have graduated from a medical school, and have obtained an ECFMG Certificate (preferably valid indefinitely, if not specific other requirement apply).  We do not have requirements concerning clinical rotations.  Another requirement is to have been accepted into a residency program by one of our two medical schools in Nebraska.

Nevada
The only licenses available are for residents in training programs

in Nevada. You must have ECFMG Certification (after graduation from Medical School) to be a

resident in Nevada.  I have not heard of any specific clinical rotation objectives.

New Hampshire
In order to obtain a training license in the state of New Hampshire, you must be a resident in an ACGME accredited institution.  I am unsure from your email if you are trying to obtain one right now or just asking for future reference.
Each program sets their own criteria in terms of acceptance.  Our program requires that you be certified by the ECFMG, have the ability to obtain a J-1 visa, have passed TOEFL, and have proven three months of U.S. clinical, hands-on, practice.

New Jersey
New Jersey requires at least 4 wks in each of the core rotations to be done at an ACGME program which has a residency program in the same area as the rotation.
New Mexico
IMG must have an ECFMG certificate to obtain a training licence.   The Board will also require a copy of your medical school transcript.
One would have to have a valid ECFMG certificate.  If they are on a visa it would require a J-1.

North Carolina
To be eligible for a NC resident training license you must have your MD degree (4 years of medical school required by law) and a current standard ECFMG certificate.  Currently the NC Medical Board does not have any requirements regarding clinical clerkships.

First of all you have to be enrolled or have signed a contract with a ND residency training program.  They (the residency coordinators) will then provide you with the application for a postgraduate training license. We do not have a requirement regarding medical school rotations.

Ohio
The Board is only concerned with the training that is obtained after you have graduated from medical school. If you are considering applying for a full, unrestricted in Ohio, and are a foreign graduate, one of the requirements are 2 years of postgraduate training. As far as a training license in Ohio, you need to be ecfmg certified and also be accepted in a training program. The rotations that you did during medical school have no bearing on a training license in Ohio.

The main requirements are graduation, ECFMG certification, and passing USMLE steps 1 & 2 within the limits set by law.  The application is available on our web site (www.okmedicalboard.org).  Just click on MD Forms & Resources.  You will need to print the application, Form #1 and Form #5.  Follow the instructions on the application form.
Effective January 1, 2004, any applicant that graduated from a foreign medical school after July 1, 2003 and completed clerkships in the United States, those clerkships must have been done in hospitals, schools or facilities that are accredited by the appropriate accrediting body such as the Accreditation Council for Graduate Medical Education.

We can provide a training license, however, usually the school that provides the residency training obtains the exemption.
The school that you will be doing your residency through provides the exemption for you.  Such as, if you are doing your residency through Vanderbilt University at the Vanderbilt hospital, Vanderbilt will apply to us for an exemption for their trainees.  Meharry College does the same thing.  The school tracks clinical rotations, etc. we do not.

Vermont
Application for a limited temporary license must include with the
application the required fee, a certified copy of the applicant’s
medical school diploma, and a supervising physician’s statement,

school that is legally chartered in the country where it is located. The

West Virginia
West Virginia does not issue a training license.  Participants in

Wyoming
Wyoming does not issue ‘training’ licenses.  In order to be eligible to apply for a license here you must have satisfactorily completed two years of postgraduate training in an ACGME or RCPSC accredited program and have passed the tests administered by the ECFMG and the USMLE.
